Transcript Request
Mercy College of Ohio’s Student Records Office maintains all of the student records for Mercy School of Nursing, St. Vincent School of Nursing, St. Vincent School of Radiography, and the Mercy College of Northwest Ohio since its inception in 1993.

To request a transcript for any of the above institutions, you must put the request in writing. To do so, you must complete a Transcript Request Form. You can either:

• submit it in person to Darnell Irvine,
• fax it to 419-251-0629 attn: Peggy Zeller, or
• mail it to:

All transcript requests are completed in 24-48 hours upon receipt. Also, there are no fees associated with transcript requests.

Enrollment Verification

Mercy College has authorized the National Student Clearinghouse to act as our agent for all* verifications of student enrollment.  Please visit the Clearinghouse online at www.studentclearinghouse.org or contact them by phone at 703-742-4200.

*Students requiring enrollment verifications to be completed for insurance purposes (health or automotive) must submit the form to Darnell Irvine at the Youngstown Campus.
