Transcript Request
Mercy College of Northwest Ohio’s Student Records Office maintains all of the student records for Mercy School of Nursing, St. Vincent School of Nursing, St. Vincent School of Radiography, and the College since its inception in 1993.

To request a transcript for any of the above institutions, you must put the request in writing. To do so, you must complete a Transcript Request Form. You can either:

  • submit it in person to Darnell Irvine,
  • fax it to 419-251-0629 attn: Peggy Zeller, or
  • mail it to:

All transcript requests are completed in 24-48 hours upon receipt. Also, there are no fees associated with transcript requests.
